No. First, diagnosis are not based solely on the basis of answers to a questionnaire. Interviews, history taking, gathering of information from others, etc. all go into making a diagnosis. Second, the tests and questionnaires that do get used to help make diagnoses are protected. That means that you have to have proper credentials to buy them, use them and interpret them. You won't find the "real" ones on line.
