What are six things that you need to think about when you are writing a questionnaire?

What are six things that you need to think about when you are writing a questionnaire?
1) Who will be filling out the questionnaire?





2) What are the most useful ways to categorize the respondents? Do you want to observe what responses are most common in men versus women? One race versus another? Different responses from different age groups?





3) What questions do you need to ask? What do you want to know?





4) Are you trying to prove something specific with your poll results? How can you phrase your questions in such a way as to get the information you want?





5) Who will be looking at your results?





6) After the results are in, what follow-up research can you do to back up what you've learned from the questionnaire?
