...I never know what to put. I am 35 years old, have a history of seizures (I take medication for it and haven't had one since 1991), have Crohn's disease (though not severe), am 5'11" and 155lbs. Normal blood pressure and cholesterol levels, have a history of kidney stones (three incidents in the past five years).
What would you say my overall health condition is?
When asked on health questionnaires regarding the state of my health (excellent, good, fair, poor)...?
Sounds like any health issues you have are under control: if it were me, I'd rate the state of my health as good, at the minimum.
