Where can I find a Secret Pal questionnaire?

I have looked all over online in search of a survey / questionnaire type form for our school to do the Secret Pal game with the teachers. All I have been able to find online is a knitting Secret Pal questionnaire that will not even come close to what I need.





I had to create one last year and it was really lacking. I even forgot to put "birthdate" in as a question. Please guide me to an existing one I can cut and paste. It does not have to be something they fill out online, just something I can photocopy and distribute to them.

It helps to know what interests or crafts you will be Secret Pal-ing. I have a Secret Pal questionnaire that my online quilting group has used many times. You can email me and I can send you a copy that you could print if you like. I will even change it as necessary to suit your specific group needs. Best of luck.
