What question would you ask in a lifestyle questionnaire?
Typical questions in a Social History for health/medical purposes include:





Do you smoke? How much? How long?


Do you consume alcohol? How much? How long?


Do you consume caffeine? How much? How long?


Do you use ilicite (street) drugs? What kind? How much? How long?


Do you sleep through the night? How long?


Do you exercise regularly? What type? How often? How long?


Are you sexually active?


How many sexual partners have you had?


Have your sexual partners been... Men Women Both?


Do you use protection from STD's?


Have you been exposed to farm animals?


(Of special interest would be chickens and other fowl, avian flu you know.)








There may also be questions concerning your living conditions:





Where do you live?


Who do you live with?


Do you live with pets?





There are many other areas that a doctor may want to know about with regard to your social habits. It could alert him to risky situations or help in a diagnosis.
